Stargardt disease is also called Stargardt macular dystrophy, juvenile macular degeneration, or fundus flavimaculatus. The disease causes progressive damage — or degeneration — of the macula, which is a small area in the center of the retina that is responsible for sharp, straight-ahead vision. In most cases, Stargardt macular degeneration is caused by mutations in the ABCA4 gene. Less often, mutations in the ELOVL4 gene cause this condition. The ABCA4 and ELOVL4 genes provide instructions for making proteins that are found in light-sensing (photoreceptor) cells in the retina.

Stage 1:

           ERG and EOG are normal

      Stage 2:

            Pisciform flecks present beyond 1 DD of the fovea, often extending beyond the arcades and nasal to the optic disc

            ERG and EOG normal, but cone/rod response may be subnormal

     Stage 3:

           Diffusely resorbed flecks and choriocapillaris atrophy of the macula

           EOG subnormal

           ERG subnormal cone or rod amplitudes

          Central field defects + midperipheral/peripheral field defect

    Stage 4:

          Diffusely absorbed flecks and choriocapillaris/RPE  atrophy throughout the entire fundus

          ERG with reduced rod and cone amplitudes

          Peripheral fields with moderate to extensive restriction

Epidemiology- Stargardt macular degeneration is the most common form of juvenile macular degeneration, the signs and symptoms of which begin in childhood. A prospective epidemiologic study (Cornish et. al 2017) undertaken under the auspices of the British Ophthalmological Surveillance Unit (BOSU) suggests an annual incidence in the United Kingdom of between 0.110 and 0.128 per 100 000 individuals. Fragile X syndrome is a genetic condition that causes a range of developmental problems including learning disabilities and cognitive impairment. Hunter syndrome, also known as Mucopolysaccharidosis II is a rare X-linked recessive disorder caused by deficiency of the lysosomal enzyme iduronate sulfatase, leading to progressive accumulation of glycosaminoglycans in nearly all cell types, tissues, and organs. Mutations in the IDS gene cause MPS II. The IDS gene provides instructions for producing the I2S enzyme, which is involved in the breakdown of large sugar molecules called glycosaminoglycans (GAGs).

Two relatively distinct clinical forms of MPS II have been recognized i.e., the non-neuronopathic form (formerly defined as slowly progressive milder form) of the disease, intelligence may be normal or only slightly impaired and neuronopathic form of the disease (formerly called early progressive more severe form), intellectual disabilities may be apparent in the early life of the patient.

Huntington disease (HD) is a brain disease that is passed down in families from generation to generation. Narcolepsy is a sleep disorder characterized by excessive sleepiness, sleep paralysis, hallucinations, and in some cases episodes of cataplexy (partial or total loss of muscle control, often triggered by a strong emotion such as laughter). Narcolepsy type 1 occurs when nearly all of the neurons that contain orexin (also called hypocretin) are lost.  The reason for this is not fully understood, but it is thought to be an autoimmune process possibly triggered by an infection. HLA haplotype DQB1*0602 is present in 95% of narcolepsy type 1 patients, but this is also present in about 20% of the general population without narcolepsy. The cause of narcolepsy type 2 is not entirely clear. Current hypotheses include less destruction of orexin cells, impaired orexin receptor signaling, or an unknown mechanism.

Narcolepsy Type 1 – This type of narcolepsy involves a combination of excessive daytime sleepiness and one or both of the following:

Cataplexy is when you have attacks that cause a sudden loss of muscle tone while you are awake. It may lead to slurred speech and buckling knees, or in more severe cases complete paralysis. These events are usually triggered by strong emotions such as joy, surprise, laughter or anger.

Low or absent CSF hypocretin-1 levels. Narcolepsy type 1 is caused by a deficiency of hypocretin (orexin). A patient with low hypocretin has narcolepsy type 1, even if they don’t exhibit cataplexy.

Narcolepsy Type 2 – This type of narcolepsy occurs when you have continuous excessive sleepiness but no cataplexy. You may take a nap for a couple of hours and wake up feeling refreshed. But after a short time, you feel tired again.

Epidemiology- according to Thelansis Narcolepsy prevalence is 14 per 100,000 people for narcolepsy type 1 and 65.4 per 100,000 people for narcolepsy type 2. Incidence is highest in the late teens to early twenties with a 50% greater female predominance in the U.S
